This series of illustrations depicts a posterior thoracic incision to access the tear of the posterior crus of the diaphragm. The prior gastroesophageal valvuloplasty is taken down, a Belsy Mark IV fundoplasty is performed and the tear of the posterior crus of the diaphragm is repaired using sutures and a dual mesh.
